How many hours of service was I authorized for?

Premier can verify the number of hours you have been authorized for by e-mail or phone.  Please contact us at customerservice@phs-west.com or by phone at 1-866-428-1730 to inquire about your hours.  

In order to better serve you, please have ready the name of the Regional Center, client and service which you are inquiring about. If you know the client's Unique Identification Number (UCI#) with the Regional Center, please include that as well.
